Otsuka offers four Anthem preferred provider plans (PPOs) with the same network. Each of the medical plans includes prescription drug coverage that corresponds with your medical plan.

  • For employees in Hawaii, Otsuka offers a single medical plan, administered by Hawaii Medical Service Association (HMSA).

The medical plans differ in how much each one pays for covered services and how much you pay for the cost of coverage (employee rates).  Your prescription drug coverage cost (employee rates) is included in your medical plan, and how you will pay for your prescriptions is based on your medical election.

About the Anthem Consumer Select and Value Plans

How the Plans Work

​

  • Once you meet your annual deductible, you pay a percentage of your healthcare costs (called coinsurance).

​

  • Only services covered by your plan count toward the annual deductible.

​

  • In-network preventive care is covered 100% and is not subject to the annual deductible.

​

  • Certain preventive care medications (e.g., for blood pressure, cholesterol, asthma, or diabetes) are covered at 100% and are not subject to the annual deductible.

​

  • These plans are paired with a Health Savings Account (HSA) in your name to which Otsuka contributes. You may also contribute to the account. Through the HSA, you save on taxes three ways: You pay no taxes on contributions (Otsuka’s or yours), withdrawals for eligible expenses, or investment earnings.​​

Benefits Eligibility

You’re eligible to participate in the Otsuka benefits program if you are a regular, full-time employee or a part-time employee scheduled to work at least 15 to 30 hours per week.

 

Eligible Dependents

You may enroll your eligible dependents for certain levels of benefit coverage. Eligible dependents include:

​

  • Your spouse or domestic partner

​

  • Your children or your domestic partner’s children, up to age 26

 

If you’re enrolling anyone other than yourself, you may be required to provide proof of their eligibility. You may not enroll any dependents without enrolling yourself in Otsuka’s plans.
